Transitions towards end-of-life can cause stress and anxiety on older adults and their families. Our goal at Bross Street is to provide the most comfortable care, in our home, to ease transitions and reduce any unnecessary anxiety during this difficult time. Our providers and clinicians are able to work alongside hospice teams to provide older adults with a place to receive end-of-life care that is personal and as close to home as possible. Each one of our residents has the option to receive end-of-life care, including support from in-home hospice, while remaining in their home at Bross Street. We also welcome residents transitioning into hospice care.
Family Planning and Support
Our team of licensed clinicians will be available on-call throughout the end of life process. We are able to support families in planning sacred transitions, including religious and final wish preferences, to ensure that the end-of-life of each person is respected. We also offer options for therapeutic services, including grief, loss, and terminal diagnoses counseling, for both families and older adults in care.
End-Of-Life Arrangements and Care
Our team is able to support and advise any needed end-of-life arrangements, including an end-of-life celebration or moment of memory at the home, to honor each resident and their families.
